How to Apply:
Final Grant Report Form

 
Recipients of grant support from the New Hampshire Charitable Foundation and its regional divisions and affiliated trusts are required to complete a final report upon completion of the grant funded project, or a status report on projects in progress, before subsequent grant applications will be considered. Recipients of multi-year grants must submit a status report on the anniversary of the original grant award date before additional funds are released. Grant recipients who do not submit final reports will not be eligible for future funding.

We rely on your candid responses to assist us in program planning. Please give us your honest thoughts about both the short-term results and the longer-term potential of the project for which funds were granted. We are as interested in what didn't work as what did, we all benefit by reflecting on lessons learned.

Your report should be based on the Project Description and Detailed Evaluation Plan in your original proposal narrative. Please refer to your original proposal as you prepare this final report.
